Yes. Chlorine takes time to kill bacteria. If you swallow the water, you may likely contract a number of diseases. Water can be contaminated by "near bowel movements", as human feces pollutes (the harmful kind of dirty) the water. If someone has diarrhea, that can also spread very quickly. If you swim underwater with your eyes open and your eyes become red/pink, become suspect of the cleanliness of the water.
Be careful with lakes and even the ocean. Runoff into the lakes and all the living micro organisms in them may not all be harmless.
I've never heard of anything serious being transmitted though, so you don't have to be super paranoid and never swim again. Just don't swallow the water.
