Communicable diseases are the ones that folks can give each other through various types of contact. Some would be HIV, common colds, tuberculosis and bubonic plague.
Non communicable diseases are ones like diabetes, rheumatoid arthritis, high blood pressure and heart diseases.
There are all sorts of ways communicable diseases are spread...sexual contact, contact with droplets (from coughing as in tuberculosis), contact with contaminated surfaces (common cold), and direct contact (bubonic plague).
