Up to how much can tests cost? Do they really need insurance information? I know I'm a minor and we can only do so much, but I want to keep this as private as possible.

You might be able to find a free clinic locally or a Planned Parenthood that will do low-cost testing. Run a search for "subsidized STD testing [your county here]" or "low-cost STD testing [your county here]" or try "free clinic [your county here]". You might have to wait for a while (free and low cost clinics are often crowded).
If you cannot find a free or low cost clinic, please just bite the bullet and tell your parents you want to be tested. It is expensive to run lab tests at your doctor's office, you are better off running it through your insurance. As the policy holders your parents will get at the very least an Explanation of Benefits in the mail for the appointment.

You can check the website hivtest.org to find free STD/HIV testing sites in your area. Testing is confidential. Results, and even the fact that you were tested, can not be released without your written permission. (Some testing centers use anonymous testing, where you would not even have to use your real name.)
